On 8.1.26, while operating my 2019 Honda Odyssey, the vehicle suffered a catastrophic, un-commanded automatic emergency braking event followed by a total failure of the brake-release fail-safe mechanisms.

The vehicle was in reverse and started to accelerate uncontrollably in reverse. The car came to a stop after hitting a wall and a car. The uncontrolled acceleration happened when we went about 1 foot onto grass while in reverse.

My car slammed on the brakes on its own. I was almost rear ended. I was going 45-50 miles an hour. I think it saw a shadow. Previously it has alerted me to brake because of shadows. This is the first time it braked hard on its own.

2025 Honda Odyssey with 21.5k miles. Noticed the van shaking when braking. Took it to the dealer for warranty service and they claim that warped rotors were the issue. Declined to provide warranty service claiming brakes are only covered for 12 months.
