I am submitting this correspondence to formally report a serious safety incident involving my 2020 Lexus RX 350 and to request a thorough investigation.
Making a left turn at night going about 10 mph I was struck by a car with no lights. He hit the right front of my vehicle going about 55-60 mph. He hit my front strut and headlights and grill and radiator.
The car does not alert when a seat belt is not worn in the back seats. This feature is mentioned in the owner's manual, but the car fails to alert when a rear passenger does not buckle their seat belt.

Summary: Toyota Motor Engineering & Manufacturing (Toyota) is recalling certain 2025 Toyota Camry Hybrid, Lexus NX250, NX350, NX350 Hybrid, 2024-2025 Lexus RX350, RX350 Hybrid, and RX500 Hybrid vehicles. The second-row center seat belt webbing may be damaged. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard (FMVSS) number 209, "Seat Belt Assemblies."
Remedy: Dealers will inspect the seat belt webbing and replace the seat belt assembly if necessary, free of charge. Owner notification letters were mailed March 20, 2025. Owners may contact Toyota's customer service at 1-800-331-4331. Toyota's numbers for this recall are 25TA03 and 25LA03.

Summary: Toyota Motor Engineering & Manufacturing (Toyota) is recalling certain 2020-2021 Avalon, Avalon Hybrid, Corolla, Highlander, Highlander Hybrid, RAV4, RAV4 Hybrid, Lexus ES350, Lexus RX350, Lexus RX450H, 2021 Sienna Hybrid, Lexus ES250, 2020-2022 Camry, Camry Hybrid, and ES300H vehicles. A short circuit may develop in the Occupant Classification System (OCS) sensor, preventing the front passenger air bag from deploying.
Remedy: Dealers will inspect and replace the sensor as necessary, free of charge. Interim letters notifying owners of the safety risk were mailed in February 2024. Some final remedy owner letters were mailed October 31, 2024. Letters will continue to be mailed in phases through January 2026. Owners may contact Toyota customer service at 1-800-331-4331. Toyota's number for this recall is 23TB15/23TA15 Lexus 23LB03/23LA03.
